You decide to quit sugar, and within about a day you hit the same fork everyone hits. Diet soda, or nothing? Stevia in the coffee, or black? Is the protein bar sweetened with erythritol a win or a cheat?
The public answer to this is unusually bad. One side treats sweeteners as poison, the other treats them as free sugar with no downside, and the actual research supports neither. What it supports is a narrower and more useful position: sweeteners are a decent transitional tool and a poor destination.
What the WHO Guideline Actually Said
In May 2023 the World Health Organization published a guideline recommending against the use of non sugar sweeteners for weight control or reducing the risk of noncommunicable diseases. The coverage read as though sweeteners had been declared harmful. That is not what happened.
Three details matter:
It was a conditional recommendation graded on low certainty evidence. In WHO language, conditional means the panel expects the right choice to vary by person and context and that policy discussion is needed. It is the weakest category they issue.
It was about effectiveness, not toxicity. The review found that non sugar sweeteners did not produce useful long term weight or disease benefits in the observational data, and that long term observational studies actually associated higher intake with worse outcomes, which the panel flagged as likely to involve reverse causation. People who are already gaining weight or already have diabetes are more likely to switch to sweeteners in the first place.
It excluded sugar alcohols. Erythritol, xylitol and the rest are low calorie sugars, not non sugar sweeteners, and were outside the scope. It also did not apply to people managing diabetes, or to products like toothpaste.
So the honest summary is: swapping sugar for sweetener does not, on its own, reliably fix weight over years. That is a claim about strategy. It says almost nothing about a single can of diet cola.
The Aspartame Headline, Decoded
Two months later, in July 2023, two WHO bodies published on aspartame on the same day and were widely reported as one story.
IARC, the cancer agency, classified aspartame as **possibly carcinogenic to humans**, Group 2B. JECFA, the food additive committee, reviewed intake and **kept the acceptable daily intake unchanged at 40 mg per kg of body weight**.
Those look contradictory because Group 2B sounds alarming and is not. IARC classifies hazard, meaning whether a substance can cause cancer under some conditions, not risk at real world doses. Group 2B is the category for limited evidence, and it contains aloe vera extract and pickled vegetables. It says the evidence is thin and unresolved, not that a dose you would encounter is dangerous.
The intake arithmetic is the part worth carrying. At 40 mg per kg, a 70 kg adult has a daily allowance of 2,800 mg. A can of diet soda contains roughly 180 to 200 mg. That is about 14 cans a day, every day, for life.
If you drink two, aspartame is not the thing standing between you and your goal.

Two lines in that table deserve expanding.
Erythritol. A 2023 paper in Nature Medicine from a Cleveland Clinic group found that higher circulating erythritol was associated with major cardiovascular events across three cohorts, and showed enhanced platelet aggregation in follow up work. It is a real finding and it changed how a lot of people treat erythritol heavy products. The interpretive catch is that humans synthesise erythritol endogenously through the pentose phosphate pathway, and production rises under the same metabolic stress that raises cardiac risk anyway. A follow up study on xylitol in 2024 reported a similar pattern. Reasonable reading: do not make sugar alcohols the base of a daily habit, and do not treat an occasional serving as a hazard.
Sugar alcohols and your gut. They are incompletely absorbed, which is why they have fewer calories and also why 30 g of them makes you regret the whole afternoon. Tolerance varies enormously between people. If a sugar free chocolate has ever ruined your evening, that is the mechanism, not an allergy.
The Question That Actually Decides It
Forget the toxicology for a moment. The variable that determines whether a substitute helps your quit is whether it interrupts the habit or preserves it.
A craving is a learned loop: a cue, an expectation, a behaviour, a reward. The 3 pm slump, the vending machine, the sweetness, the relief. Sugar is the reward but it is not the loop.
Swap the sugar for a sweetener and you have changed one component. The cue still fires, the ritual still runs, the sweet expectation still gets confirmed. Sometimes that is exactly what you need, because a change that costs you nothing is a change you will keep. And sometimes it means that six weeks in you are just as dependent on the ritual and one bad day away from the real thing.

The taper works better than the switch. Sweeten less each week rather than swapping the source, because the target is a lowered sweetness expectation, and that only moves if the sweetness actually comes down. Practical Rules
1. **Kill liquid sugar first, by any means.** Diet drinks are an acceptable tool here. This is where the calories are and where the substitution evidence is strongest.
2. **Do not sweeten the things you can learn to like unsweetened.** Coffee, tea, yoghurt and porridge all recalibrate within a few weeks. Sweetening them permanently locks the expectation in place.
